Application forms

The Alberta Provincial Nominee Program (PNP) Application Forms required to apply in the Skilled Worker Category are:

Employer and Worker Application for a Skilled Occupation (ABPNP 005)

This form must be completed together by the Employer and the Provincial Nominee Candidate.

Application for Provincial Nomination (ABPNP 003)

This form must be completed by the Provincial Nominee Candidate if the Employer and Worker Application for a Skilled Occupation (ABPNP 005) has been completed.

Both applications described above, and their supporting documents, must be submitted to the Alberta PNP at the same time to apply in the Skilled Worker Category.

Citizenship and Immigration Canada (CIC) Forms

These forms must be completed by the Provincial Nominee Candidate with the Application for Provincial Nomination (ABPNP 003). Only a photocopy of the CIC Forms is required for submission to the Alberta PNP.
